Teacher Fellowship Agreements

The following are the basic requirements for the CSME Teacher Fellowship. We also anticipate that each of you will contribute to the community in unique ways, and will be supportive of each other’s work.

Members

As a CSME Teacher Fellow Program Member I agree to meet the following requirements:

  • Complete evaluations or written reflections as assigned to me
  • Attend CSME Teacher Community Meetings (Must stay for the entire program and attend the events as indicated below based on your content area)

Undergraduate Fellows

As a CSME Undergraduate Teacher Fellow I agree to meet the following requirements:

  • Meet for two individual meetings with CSME Teacher Program director/CSME faculty
  • Meet with my academic advisor during the semester to confirm graduation date
  • Apply for at least ONE scholarship, internship, or fellowship
  • Do a minimum of 40 hours Early Field Experience (to be arranged with program director)
  • Complete evaluations or written reflections as assigned to me
  • Attend ALL CSME Teacher Community Meetings (Must stay for the entire program and attend the events as indicated below based on your content area)

Credential Fellows

As a CSME Credential Fellow I agree to meet the following requirements:

  • Agreement to host a CSME Teacher Fellow in your classroom
  • Complete evaluations or written reflections as assigned to me
  • Do a minimum of three hours of CSME Teacher Outreach (TBD with program director)
  • Attend ALL CSME Teacher Community Meetings (Must stay for the entire program and attend the events as indicated below based on your content area)

Reimbursement Form

CSME will reimburse for various services related to your teacher preparation (CBEST, CSET, Credential Application to SFSU, Livescan, etc.). These funds are available on a first come, first served basis. Please pick up a copy of the reimbursement form in SCI 211.

Livescan Fingerprinting Instructions (On Campus at SF State)

In order for the fellowship to fund your livescan, please follow these instructions:

  1. Make an appointment with Campus Police at 415-338-6043. When you are speaking with the representative, please tell her/him you are from the CSME Teacher Fellowship and that you are on our student list. This ensures that they bill the livescan service to us (CSME) and that you do not have to pay for it up front.
  2. Please be sure to bring the proper documentation and paperwork to the livescan appointment. In most cases, this includes the San Francisco Education Fund form which was provided to you during the San Francisco Education Fund training. Note: Bringing the proper paperwork and documentation is essential. Without it, your information will not be sent to the proper place (which would be the San Francisco Education Fund).
  3. After you get fingerprinted, make sure you get some form of receipt or proof of completion (a duplicate copy of the completed livescan form will suffice).

Coursework Evaluation

If you want to have your existing undergraduate coursework evaluated for fulfilling math competency equivalent to a single-subject waiver program, please follow these steps:

1. Unless you are a current SFSU student or applying for the SFSU credential program, send or hand-deliver a check for the evaluation fee of $100 made out to SFSU, to

Write "transcript evaluation" in the comment field. This fee applies whether or not you have previously started the process with Prof. Marcucci. This fee will only be charged once per student. There may be an additional credential office fee for a certificate ($25 as of June 2010).

2. Download the form Coursework List, filling out a proposed course by course equivalence between your CSU coursework and the following 45-unit SFSU waiver program, and e-mail it to erichsu@sfsu.edu.

We are allowed to use some judgment, so please list separately at the end any other math courses you've passed that have no precise SFSU equivalent.

3. Provide an electronic copy of a transcript with final grades for the courses you list.

4. Provide official course descriptions for any non-SFSU courses you list.  A web link to an official university course description site is sufficient, if it tells us what was actually covered in the courses taken.
